Information on Mailing or Emailing the Forms

Printing the Forms

If you are interested in sponsoring a high school teen (ages 15-18) or if you desire to attend a Chrysalis Flight, below are links to both the butterfly and the sponsor forms.

Both forms are to be completed and mailed or sent via email together.

Gentle reminder - if the butterfly is a minor (not 18 years of age) then the registration form must be signed by a parent.

The Registration and Sponsor forms may be printed, filled out and mailed to either address below:

Once the forms have been completed they can also be scanned and then emailed to the registrar.

If you do email the forms, you should still mail the $15.00 deposit fee (with the name of the butterfly and flight # clearly indicated) to the NWO Chrysalis Registrar at the address provided above.

Select the option
"print selection."

This will allow you to avoid printing the unwanted banners.
